5) ensure pt signs consent form before tx

Biopsy (bx)
performed to determine definitive diagnosis of a lesion
templates: “*BX - new patient” or “*Bx - returning pt”
have pt sign consent form prior to bx
take photo of lesion on an office cell phone after MD or PA has circled the lesion
use a single bx burrito, located in the top drawer of the back cabinet in each exam room; open up burrito on pull-out tray
grab biopsy bottle from back cabinet
one bottle per biopsy (2 biopsies = 2 bottles)
in similar locations, provider will label sites A, B, …
hibiclens on a piece of gauze and place on tray for MD/PA to clean site
label each bx bottle with pt labels located on the bottom shelf of the back cabinet SEPARATE LABEL FOR EACH BIOPSY PERFORMED
prepare bandaging materials (small amount neosporin, band-aid
contents of bx burrito
1) 1 cc lidocaine
multiple biopsies = grab larger lidocaine
2) 3 pieces of gauze
3) 3 CTA’s
4) 1 blade
