AcP C (ACP Comfort)
Focuses on symptom control and comfort care.
AcP M (ACP Medical)
Involves accepting all medical interventions except resuscitation.
AcP R (ACP Resuscitation)
Relates to decisions regarding resuscitation.
SOMETEAMS
An acronym used for a general survey when entering a patient's room: Symmetry, Old, Mental Acuity, Expression, Trunk, Extremities, Appearance, Movement, Speech.
Symmetry (in SOMETEAMS)
Assessing if the patient's face and body are symmetrical.
Old (in SOMETEAMS)
Assessing if the patient looks their stated age.
Mental Acuity (in SOMETEAMS)
Assessing if the patient is alert, confused, agitated, or inattentive.
Expression (in SOMETEAMS)
Assessing if the patient looks ill, in pain, or anxious.
Trunk (in SOMETEAMS)
Assessing the patient's body build (stocky, lean, obese, barrel chested).
Extremities (in SOMETEAMS)
Assessing the colour, condition of skin, and deformities of the limbs.
Appearance (in SOMETEAMS)
Assessing if the patient is dressed appropriately and is clean.
Movement (in SOMETEAMS)
Assessing the patient's posture, gait, and coordination.
Speech (in SOMETEAMS)
Assessing if the patient's speech is relaxed, clear, strong, comprehensible, appropriate, or stressed.
Study of a whole person (General Survey)
An introduction for a physical examination, giving an overall impression of the patient from first sight.
Bedside Safety Check
A routine assessment including distress, level of consciousness (LOC), environment, patient identification/allergies, ACP, bed position, room conditions, tubes/IVs/dressings, functioning O2/suction, and call bell reach.
Types of Infection Control
Standard and Isolation (also called transmission-based).
Standard Infection Control
Basic infection prevention measures including hand hygiene (HH) and personal protective equipment (PPE).
ABC BD of dignity
An acronym for Attitude, Behavior, Compassion, and Dialogue, promoting patient dignity.
Donning PPE
The sequence for putting on PPE: Gown (neck, back tied), mask, eye protection, gloves (over the cuff).
Doffing PPE
The sequence for taking off PPE: Gloves, Hand Hygiene (HH), gown, HH, eye protection, HH, mask, HH.
Alcohol Hand Hygiene
Used before and after patient care, for 15 seconds, and is better for the skin.
Soap Hand Hygiene
Used when hands are visibly soiled or when caring for patients with C. difficile; performed for 40-60 seconds and pat dry.
Contact Isolation
Measures for infections like C. difficile, wounds, and skin infections, requiring gloves and gowns.
Droplet Isolation
Measures for infections like rubella, influenza, and mumps, requiring a mask and eye protection when within 2 meters of the patient.
Airborne Isolation
Measures for infections like measles and chickenpox, requiring a closed door, negative pressure room, and an N95 mask.
Bed Bath
A procedure for washing a patient in bed, typically performed with the patient supine using a bath blanket, following a specific order for washing body parts.
Oral Care Angle
The recommended angle for brushing the gum line, which is 45 degrees.
Periodontitis
A condition characterized by receding gum lines.
Halitosis
The medical term for bad breath.
Cheilosis
A condition involving cracking of the lips.
Stomatitis
Inflammation of the oral tissue.
High-risk patients for Nail & Foot Care
Include older adults, individuals with diabetes mellitus, heart failure, renal disease, or a history of stroke.
Nail & Foot Care Guidelines
Involve soaking feet and nails for 10-20 minutes (unless diabetic); nails should only be filed, not cut, if the patient has circulatory problems.
Indwelling Catheter Care
Involves standing on the patient's left side and recording output at least every 8 hours.
