The Apical Window


is identified initially by palpating of the LV apex with the patient in steep LLD position. Placement of the probe on the Apical Impulse (PMI)- The most Lateral and Inferior point over the Apex where the Cardiac Impulse can be felt.

(Then angle Superior and Medial toward the Heart).

The AML is adjacent to the Septum

The PML is adjacent to the Lateral wall.

The TV annulus is closer to the apex (up to 1 cm).

Septal leaflet – is located adjacent to the septum.

The Anterior Leaflet is the other leaflet seen adjacent to the free wall.

The DAO may be seen Lateral to the LA.

The PV4 enter the LA Posteriorly but may be difficult to see at this depth in adults.

If probe is angled posteriorly from the 4CH view, the Coronary Sinus is seen in the Atrioventricular groove.

Angle the transducer more Anterior from the Ap4ch to acquire the apical 5 chamber (Ap5ch) view.

This opens up the LVOT and Aortic Valve. (hence the 5th chamber)
