What are the minimum altitudes for IFR according to 91.177?
Cant operate below MEA and a MOCA for a particular route segment, can go below MEA down to but not below MOCA. For VOR nav this is within 22nm of VOR. Climbs to higher minimums should begin immediately after passing beyond which the min. applies. The point beyond which that higher min. alt. applies shall be crossed at or above the applicable MCA.

In controlled airspace, altitude is whatever is assigned by ATC. In uncontrolled when below 18000msl on a mag course of 0-179 any odd thousand-foot msl altitude. On mag course 180-359 any even thousand foot.

Must fly on ATS route along the centerline of the runway. On any other route along the direct course, but does not prohibit maneuvering to pass clear of air traffic or maneuvering to VFR conditions.

If the clearance limit fix is not a fix for the beginning of an approach leave the limit at EFC time and proceed to fix that begins an approach.
According to 91.187 what do you need to report in controlled airspace regarding malfunctions?
If in controlled airspace PIC shall report as soon as practical to ATC of any malfunctions of navigation, approach, or communication equipment. In the report it should include aircraft identification, equipment affected, degree to which the capability of the pilot to operate under IFR, nature and extent of assistance desired from ATC.
