What is case management

the coordination of services. case managers are RN’s but they integrate all of the healthcare professions to support patient needs throughout the continuum of care.

what do case managers do?

They assess client needs, develop care plans, and coordinate resources, ensuring individualized care to maintain self-management for the patient.

what type of services does home care provide?

home care provides medical and non-medical services to patients across the life-span. home care nurses meet patient’s needs including health promotion, end-of-life care, rehabilitation, medication management etc.

What are some of the challenges that rural care faces?

lack of access to supports, inadequate transportation, inadequate resources, lack of healthcare providers, protecting patient privacy, as well as addressing specific needs of the community.
