rights of medication administration
•six basic rights of medication administration right patient, drug, dose, route, time, documentation
•A violation of any of the 6 basic rightz constitutes a medication error.
additional rights include right reason, right to know , right to refuse, right techniques
right patient
•Check the name and date of birth on the order for a drug or vaccine and the patient's chart
•Then compare to the name and the date of birth that the patient tells you
•Do not call the patient by name because I forget for confused patient may answer to any name
•Have an attending caregiver or family member state the name and date of birth if the patient is unable
right drug
always perform triple check when preparing a medication
•Compare name of prescribed drug or vaccine in the patient's chart with the label on the container,
•Then look at the expiration date, never use if pass this date
•If unfamiliar w/drug, use credible drug reference
•NEVER PREPARE A DRUG FROM A CONTAINER WITH A DAMAGED OR HANDWRITTEN LABEL

•Come have a dose on the order in the patient's chart with the does she prepare
•To obtain the right dose read the label closely and calculate accurately
•Do not confuse it goes contained in one tablet with the number of pallets in the container
right route
•Double check to male sure the administration route you are preparing to use matches the route the doctor ordered.
•Check that the medication you are using can be administered by the right order.
•Do not confuse ear otic optic drops with eye optic drops.
•check that the patient can recieve the drug by this route,route seems appropriate.
•ex: patient has an injury at the specific injection site, consult the doctor for possible alternative site or a different route

•give the drug at the right time
•must be given after meals, make sure the patient has eaten recently
•for certain drugs, ensure that it is correct time of day and the correct time in a series of doses

•document the procedures immediately after administering the drug or vaccine to the patient.
•DO NOT WAIT UNTIL LATER AND DO NOT DOCUMENT BEFORE ADMINISTRATION

•if the drug is a controlled substance document it on the controlled substance inventory record
•double check your entry for computer documentation before submitting
•use neat handwriting for written documentation

all patients have the right to be educated about the medication they are receiving includes the reason, effect and the side effects of medications

•all patients have the right to refuse medication
•if a patient refuses medication or vaccines report that physician who ordered the medication
•a refusal of medication by the patient should be documented in the patient's medical record

•if you have not giving a drug or vaccine by the ordered route recently,review the technique before administering the drugs
