Cystic

Wider-than-tall

A thyroid nodule shape feature that is assigned 0 points.

Taller-than-wide

A thyroid nodule shape feature that is assigned 3 points.

Smooth

A thyroid nodule margin feature that is assigned 0 points.

Ill-defined

A thyroid nodule margin feature that is assigned 0 points.

Lobulated or irregular

A thyroid nodule margin feature that is assigned 2 points.

Extra-thyroidal extention

A thyroid nodule margin feature that is assigned 3 points.

None or large comet-tail artifacts

An echogenic foci feature that is assigned 0 points.

Macrocalcifications

An echogenic foci feature that is assigned 1 point.

Peripheral (rim) calcifications

An echogenic foci feature that is assigned 2 points.

Punctate echogenic foci

An echogenic foci feature that is assigned 3 points.

TR1

A benign level associated with 0 points where no FNA is recommended.

TR2

A level classified as not suspicious (2 points) where no FNA is recommended.

TR3

A mildly suspicious level (3 points) where FNA is recommended if the nodule is $\geq 2.5\,cm$ and follow up if $\geq 1.5\,cm$.

TR4

A moderately suspicious level (4-6 points) where FNA is recommended if the nodule is $\geq 1.5\,cm$ and follow up if $\geq 1\,cm$.

TR5

A highly suspicious level (7 points or more) where FNA is recommended if the nodule is $\geq 1\,cm$ and follow up if $\geq 0.5\,cm$.

TI-RADS

A classification system used to level thyroid nodules by adding points from composition, echogenicity, shape, margin, and echogenic foci.

FNA

Abbreviation for Fine Needle Aspiration, a procedure recommended based on TI-RADS levels and specific size thresholds such as $\geq 1\,cm$ for TR5.
