anterior surface of humerus to coronoid process of ulna; musculocutaneous n- flexion of forearm 

Triceps brachii, medial head

posterior surface of humerus below the radial groove to the olecranon process of ulna; radial n- extension of forearm

Triceps brachii, lateral head

posterior surface of humerus above radial groove to olecranon process of ulna; radial n- extension of forearm

Triceps brachii, long head

infraglenoid tubercle of scapula to olecranon process of ulna; radial n- extension of forearm, extension of arm

Pronator teres

medial epicondyle of the humerus, coronoid process of the ulna to the lateral side of the middle of radius; median n- pronation and flexion of forearm

Flexor carpi radialis

medial epicondyle of the humerus (more medial than pronator teres) to 2nd metacarpal; median n- flexion of hand, abduction of hand

Palmaris longus

medial epicondyle of the humerus to the palmar aponeurosis (sheath of skin covering the palm); median n- flexion of hand, tightening of flexor aponeurosis

Flexor carpi ulnaris

medial epicondyle of humerus to pisiform, hamate, 5th metacarpal; ulnar n- flexion and adduction of hand

Flexor digitorum superficialis

medial epicondyle of humerus to middle phalanx of digits 2-5; median n- flexion of proximal interphalangeal joints, flexion of metacarpophalangeal joints, flexion of wrist

Flexor pollicis longus

 radius, interosseous membrane to distal phalanx of digit 1; median n- flexion of distal phalanx of digit 1

Flexor digitorum profundus

anterior ulna, interosseous membrane to distal phalanx of digits 2-5; medial half is ulnar, lateral is median- flexion of distal and proximal interphalangeal joints, flexion of metacarpophalangeal joints, flexion of wrist

Pronator quadratus

distal ulna to distal radius (wristband like); median n- pronation of forearm

Brachioradialis

lateral supracondylar ridge to distal radius; radial n- flexion and supination of forearm

Extensor carpi radialis longus

lateral supracondylar ridge to base of 2nd metacarpal; radial n- extension and abduction of hand

Extensor carpi radialis brevis

lateral epicondyle of humerus to 3rd metacarpal; radial n- extension and abduction of hand

Extensor digitorum

lateral epicondyle of humerus to extensor expansion of digits 2-5 (splits into tendons); radial n- extension of metacarpophalangeal joints

Extensor digiti minimi

lateral epicondyle of humerus to extensor expansion of digit 5; radial n- extension of metacarpophalangeal joints, extension of interphalangeal joints of digit 5

Extensor carpi ulnaris

lateral epicondyle of humerus to 5th metacarpal; radial n- extension and adduction of hand

Supinator

lateral epicondyle of humerus, ulna to shaft of radius (proximal to pronator teres); radial n- supination of forearm

Abductor pollicis longus

posterior ulna, radius, interosseous membrane to base of first metacarpal; radial n- abduction of digit 1, extension of digit 1 at carpometacarpal joint 

Extensor pollicis brevis

radius, interosseous membrane ro proximal phalanx of digit 1; radial n- extension of digit 1 at metacarpophalangeal joint
