Apixaban (Eliquis)

Factor Xa inhibitor anticoagulant; used for AFib/DVT/PE. BIG risk = bleeding. Report blood in urine, black/tarry stools, or unusual bleeding.

Tamsulosin (Flomax)

Alpha blocker for BPH. BIG risk = orthostatic hypotension/dizziness. Change positions slowly; take 30 min after the same meal each day.

Ampicillin/Sulbactam (Unasyn)

Penicillin + beta-lactamase inhibitor antibiotic. Watch for allergy/anaphylaxis and severe diarrhea/C. diff.

Cefazolin (Ancef)

Cephalosporin antibiotic; commonly used for surgical prophylaxis. Watch for allergy and severe diarrhea/C. diff.

Metronidazole (Flagyl)

Antibiotic for anaerobic infections/C. diff. NO ALCOHOL during treatment and for 3 days after.

Piperacillin/Tazobactam (Zosyn)

Penicillin + beta-lactamase inhibitor antibiotic. Watch for allergy/anaphylaxis and diarrhea/C. diff; monitor renal function.

Cefepime (Maxipime)

Cephalosporin antibiotic. BIG concern = neurotoxicity; monitor neurologic and renal status.

Ceftriaxone (Rocephin)

Cephalosporin antibiotic used for many infections including meningitis. Watch for allergy and severe diarrhea/C. diff.

Famotidine (Pepcid)

H2 blocker; decreases stomach acid. Used for GERD/ulcers/GI bleeding.

Thiamine (Vitamin B1)

Vitamin B1; used for deficiency, alcohol withdrawal, and Wernicke-Korsakoff syndrome.

Ketorolac (Toradol)

NSAID for short-term moderate-severe pain. BIG risks = GI bleeding + kidney injury. Maximum therapy = 5 days.

Methocarbamol (Robaxin)

Muscle relaxant for muscle spasms. Causes drowsiness/dizziness; avoid alcohol/CNS depressants and use caution driving.

Omeprazole

PPI. Take 30-60 minutes BEFORE breakfast; do not crush/chew delayed-release form.

Ipratropium/Albuterol

Bronchodilator combination. Expected albuterol effects include fine hand tremor and racing/pounding heart.

Warfarin

Anticoagulant monitored with INR. Therapeutic INR = 2-3 for conventional warfarin therapy.

reversal → Vitamin K (phytonadione)

Heparin

Anticoagulant monitored with aPTT. Therapeutic aPTT = 1.5-2.5 × normal.

reversal → Protamine sulfate
