Nursing Diagnosis
A label for a problem resulting from a medical diagnosis that identifies patient problems and results from an assessment of patient needs.
Nursing Process
A decision-making framework used to determine patient needs and care, providing a structure for organizing nursing care.
Expected Outcomes
Statements of measurable actions for the patient that focus on what the patient is expected to achieve and help determine if goals are being met.
Nursing Goal
The component of a care plan that provides overall direction for improving a problem and describes the overall improvement desired.
Six Vital Signs
The group of measurements consisting of temperature, pulse, respirations, blood pressure, oxygen saturation, and pain.
Objective Data
Measurable facts or observations, such as a temperature of 101.2∘F.
Subjective Data
Information based on patient opinion or reports, such as a patient stating, "I feel dizzy" or reporting a pain level of 8/10.
Pulse
The measurement of the number of heartbeats per minute.
Tachycardia
A pulse rate in an adult described as being 110beats/minute.
Bradycardia
A pulse rate in an adult described as being 52beats/minute.
Respirations
The assessment of a patient's breathing rate.
Bradypnea
A respiratory rate of 8breaths/minute.
Tachypnea
A respiratory rate of 28breaths/minute in an adult.
Blood Pressure
The measurement of the pressure of blood against the walls of the arteries.
Systolic Pressure
The first or top number in a blood pressure reading.
Diastolic Pressure
The bottom number in a blood pressure reading.
SpO2
The measurement of oxygen saturation.
Pulse Oximeter
The device commonly used to measure oxygen saturation.
Pain
A vital sign that is defined as whatever the patient says it is.
0−10 Numeric Scale
The pain rating scale commonly used for an alert adult who can communicate.
