Database
The combined collection of subjective data, objective data, the patient's record, and laboratory studies that forms the foundation for assessment.
Diagnostic Reasoning
The process of analyzing health data and drawing conclusions to identify clinical diagnoses.
Cue
A piece of important information in diagnostic reasoning used to help identify clinical problems.
Nursing Process
A 5-step method consisting of assessment, diagnosis, planning, implementation, and evaluation.
First-Level Priority Problems
Clinical issues that are emergent, life-threatening, and immediate in urgency.
Second-Level Priority Problems
Clinical issues that are next in urgency, requiring prompt attention to avoid further deterioration.
Third-Level Priority Problems
Health issues that are important to the patient's health but can be addressed after more urgent problems are resolved.
Evidence-Based Assessment
Clinical decision-making integrating the best research evidence, clinical expertise, clinical knowledge, and patient values and preferences.
Complete Total Health Database
A database that describes current and past health state and forms a baseline to measure all future changes.
Focused or Problem-Centered Database
A mini database with a smaller scope and more focused objective than a complete total health database.
Follow-Up Database
A database used to evaluate the status of all identified health problems at regular and appropriate intervals.
Emergency Database
A rapid collection of data compiled concurrently with lifesaving measures.

Holistic Model
A comprehensive healthcare framework that considers the mind, body, heart, and soul as interconnected components of overall health.

Social Determinants of Health
Factors influencing health including social support networks, working conditions, physical environment, income and social status, education, genetic endowment, coping skills, and health services.
Clinical Intuition (Hunch)
An important component of critical thinking that should be substantiated through a collaborative approach with other team members rather than acted on alone or disregarded.
