Modifier 25

The doctor did two completely separate things during this single visit, so please pay us for both. The patient visits doctor for E/M evaluation and management visit and during this visit a procedure was performed. If you are trying to get paid for a regular office visit code alongside a procedure.

Modifier 51

The doctor performed multiple procedures during this single visit, so please apply the standard discount to the extra procedures. The discount applies to the second procedure or any other procedures because the operating room was already set up and did not need to be prepped for the other procedures.

Modifier 59

These two procedures look like they are part of the same job, but they were actually completely separate and distinct, so please pay for both at full value. If you are trying to get paid for two different surgical/test codes on the same day

What happens at an E/M visit

1. Evaluation (Listening & Examining): They ask about your symptoms, look at your medical history, and perform a physical exam (like listening to your heart or looking in your throat).

2. Management (The Plan): They decide what to do next. This includes writing prescriptions, ordering bloodwork, referring you to a specialist, or telling you to rest.

3. Medical Decision Making (MDM): This is the brainwork. How sick is the patient? How risky or complicated is their condition?
