patient of yours was admitted previously with a worsening chronic heart failure. Other
comorbidities include DM2, hypertension and dyslipidemia with appropriate medical treatment.
After a tooth removal the dentist has treated her with clindamycin for the last 7 days. Now she
has developed watery diarrhea (3x/d), is dehydrated and presents high fever. The laboratory
examination shows an increase in CRP and Leukocytosis.

After it remained she decided to see a doctor.
After asking she tells you that she has been sexually active and has not been to her last
screening at the gynecologist.

oriented in terms of time, place and his person. After talking to the parents they mention that
their son had an ear infection 5 days ago, but they decided not to go to the doctor, since they
did not feel like it was serious. The boy is still complaining about mild ear pain.

examination you notice a reddish color of the skin around her central line. After you removed
the catheter and the patient developed a slight fever you decided to start an empirical
broad-spectrum antimicrobial therapy. A day later you get back the results from the submitted
catheter tip: VRE. Also the blood cultures are + for VRE.

A 56 y.o. a man returned from his camping vacation in the forest 9 days ago. Over the
last few days he started having a headache with fever, bone ache, macules and petechiae
located mostly at the ankles and wrists.
Doxycycline and Chloramphenicol
An 80 y.o. a woman from a nursing home with a 2 week hospital stay now complains
about dysuria and urinary frequency after the removal of her Foley catheter. Even though
flowers in vases are not allowed in a patient's room at your hospital, her family decided to bring
several, which have stayed in her room over the last 2 weeks. The urinary dipstick test
indicates a UTI with high leukocytes, hematuria and positive for nitrates. Urinary culture reveals
a G- oxidase and catalase positive motile rod forming a greenish culture medium that has a
linden-tree smell.
