Long Term Care (LTC)

Acute Care
24-hour skilled care given in hospitals and ambulatory surgical centers. it is for people who need short-term and immediate care
Subacute Care
care given in hospitals or long-term care facilities. it is for people who need care less than acute but more than chronic illness
Outpatient Care
care for people who have had treatments, procedures, surgeries, or need short-term skilled care that do not require an overnight stay
Rehabilitation
care given by specialists or professionals that help restore or improve function after an illness or injury
Hospice Care
given in facilities or homes for people who have 6 months or less to live
Activities of Daily Living (ADL)
daily personal care tasks
bathing, walking, eating, etc
Person-Centered Care
care that revolves around the resident and promotes their individual preferences, choices, dignity, and interests
Trauma-Informed Care
approach that recognizes that the patient may have experienced trauma in their lives
Medicare
federal health insurance for people 65 or older or with permanent kidney failure or certain disabilities
part a = helps pay for care in a hospital, skilled nursing facility, home health agency, or hospice
part b = helps pay for medical services and equipment
part c =allows private insurance companies to provide Medicare benefits
part d = helps pay for prescriptions for treatments
