Departmental Vehicle Accident
Any incident involving damage, injury, or loss caused by a vehicle operated, maintained, fueled, or insured by the Prince George’s County Fire/EMS Department.
Major Departmental Vehicle Accident
A vehicle accident that involves a fatality or serious injury, causes damage exceeding one-third of the vehicle’s value or $10,000, or is deemed unusual/unique by the Duty Chief.
Critical Accident Support and Hindsight (CRASH) Team
A Fire Chief–appointed team that provides on-scene support and conducts investigations of major departmental vehicle accidents.
Vehicle Accident Report (PGC Form #555)
Primary form completed by the driver to document details of any Fire/EMS departmental vehicle collision.
Notice of Loss or Damage Report (PGC Form #556)
Form used to report loss, damage, or theft of any County property, including vehicles.
Accident Review and/or Theft Report (PGC Form #2842)
Detailed report outlining circumstances, negligence assessment, disciplinary action, and prevention steps for accidents or thefts.
Supervisor’s Accident Investigation & Report for Vehicles (PGC Form #1385)
Comprehensive investigation form completed by the supervisor that analyzes causes, contributing factors, and corrective actions.
Additional Vehicle Accident Information Report (PGC Form #3030)
Supplemental form capturing driver status, license, radio ID, and a breakdown of estimated damages.
Inter-Office Memorandum – “Confidential in Anticipation of Litigation”
Written, signed statements from crew/passengers sent to the Associate County Attorney within 48 hours after an accident.
Risk Management Office (RMO)
County office that receives all accident documentation, forwards data to insurers, and tracks compliance.
Apparatus Maintenance
