Morse Fall Risk assessment
A scale used to determine fall risk; interventions include keeping the bed low and locked with 2-side rails up.
Braden scale
A tool used to determine the risk for skin breakdown, where a higher score indicates a better outcome.
Columbia Suicide Severity Scale
An assessment used to determine if a patient needs further evaluation or 1:1 supervision.
Initial assessment
A comprehensive admission assessment.
Reassessment
A shift or routine assessment conducted by the nurse.
Focused assessment
A problem-based assessment used for special circumstances like chest pain or trouble breathing.
Rapid 60 second assessment
A quick general survey safety check performed while introducing oneself, checking the IV, call bell, and bed position.
Comprehensive assessment
An extensive review including HPI, PMH, PSH, medication reconciliation, screenings, and a head-to-toe assessment.
QSEN
Quality & Safety Education for Nurses; outlines Knowledge, Skills, and Attitude competencies and encourages interprofessional collaboration to improve patient outcomes.
QSEN CORE Competencies
The six key areas of focus: pt-centered care, teamwork & collaboration, evidence-based practice, quality improvement, safety, and informatics.
National Patient Safety Goals (NPSG)
National priorities to maximize patient safety, including pt identification (2 Pt ID's), medication safety, and infection prevention.

Metrics used for measuring and evaluating outcomes
