Region 11 Mailing Address Change
Any change of mailing address must be reported to the appropriate Resource Hospital and IDPH within 72 hours.
IDPH License Opt-out (March)
Members must submit a request via SharePoint during the month of March to take effect on July 1st.
IDPH License Opt-out (October)
Members must submit a request via SharePoint during the month of October to take effect the following February 1st.
License Renewal Notice
All licensed members receive a renewal notice containing a PIN from IDPH 60 days prior to license expiration.
Failed Licensure Leave of Absence
A member failing to maintain an EMT or Paramedic license as a condition of employment may be placed on unpaid leave for up to 20 months.
License Expiration Notification (Non-condition)
Members voluntarily letting their license expire must submit Form 2/2A no later than 30 days prior to expiration.
OSFM Web Access Portal Deadline
All CFD members must have created and maintained a valid account on the OSFM Web Access Portal prior to June 1, 2022.
OSFM Certification Deadline
Members must complete all requirements and pass exams within 1 year of completing an OSFM-approved course.
OSFM Examination Request
All requests for OSFM examinations must be submitted at least 30 days prior to the requested date.
Immediate Family Definition
Includes spouse, domestic partner, parents, children, siblings, in-laws, grandparents, grandchildren, stepparents, and step/half-relatives.
OSFM Liaison Contact Window
Members should contact the Department OSFM Liaison if a new certification is not posted 6 weeks after passing an exam.
Category 1-4 Training Requirements
Engines, Tower Ladders, Aerials, and Squads require 12 hours of driving and 8 hours of operations training.
Discrimination/Harassment Filing Period
A complaint must be filed within 1 year of the date of the last alleged act.
Military Leave (Less than 31 days)

Members hospitalized or convalescing from military-related injury have up to 2 years to recover before applying for reemployment.
Concurrent Compensation
During annual military training, members receive full compensation for up to 30 days per calendar year.
Differential Compensation
The pay due when a member’s military daily rate is less than their daily rate of compensation as a CFD member.
Privacy Room Personal Use Limit
A member using a Privacy Room for purposes other than expressing breast milk may not exceed 2 hours.
Class D Optional Work Shorts
Authorized from April 1 through October 31 for Bureau of Operations members below the rank of Battalion Chief.
HandPunch 3000 Repair Inquiry
If a work order is not completed within 7 calendar days, supervisors must contact the Bureau of Logistics.
Bed Bug Disinfection Protocol
Impacted furniture and equipment are disinfected with 90[\text{%}] alcohol; apparatus are heated on high for 20 minutes.
Company Journal Replacement
A new Company Journal must be requested when 15 or fewer pages remain.
Fuel Level Policy
Fuel level must be maintained at no less than 21 full, increasing to 85 full during cold weather operations.
