Moderate decline. Where everyone notices and the pt goes to receive help. The doctor will give them long division problems to see what's wrong. They also forget to pay bills & become moody ad start to notice that something is wrong.

Moderately severe decline: Cognitive impartible (Shows how you think). The pt now needs assistance in living, they cannot tell you where they live, confused abt time & place. During this ask: Who are you, time & place & year. Cannot do basic math problems but remembers their own name

Severe Decline: Need help with ADL, forgotten recent events. Cannot remember their own name, hard time controlling bowl & bladder. They also do alot of wandering

Very Severe Decline: unable to communicate with you, need help with eating & using the restroom. Muscles become stiff and difficulty swallowing.
