HMOs
You can only go outside the network for emergencies. You require a referral from a primary care physician for specialists. There are usually no deductibles, and if there are, they are low. It has the lowest cost but the least flexibility
PPOs
You can go outside the network at a higher cost. There is no referral needed for out of network care, but it costs more. There are usually higher deductibles. It is the most expensive plan but the most flexible
EPOs
You cannot go outside the network unless for an emergency. There is no out of network coverage, so no referrals needed. There are lower deductibles than PPOs. Cheaper than PPO but more expensive than HMO.
POS
You can go outside of the network for a higher price. A referal is needed for out of network care. There are moderate deductibles.
Premium
The monthly/yearly cost of insurance/the bill
