what is placenta previa?
an improper implanted placenta in the lower uterine segment near or over the internal cervical opening
types: total, partial, marginal
what is total (complete) placenta previa?
the cervical os is covered entirely by placenta when cervix is dilated fully
what is a partial placenta previa?
the lower border of the placenta is within 3cm if the cervical os but does not totally cover it
what is marginal (low-lying) placenta previa?
placenta is implanted in the lower uterus but the lower border is more than 3 cm from cevical os
what are clinical manifestations of placenta previa?
sudden onset painless bright red vaginal bleeding (last half of pregnancy)
uterus is soft, relaxed, nontender
fundal height may be more than expected for gestational age
what are interventions for placenta previa?
monitor maternal VS, fetal heart rate and fetal activity
prepare for ultrasound to confirm diagnosis
do not perform vaginal examination
maintain sidelying position
monitor bleeding and treat shock
admin IV fluids, blood products, tocolytic
