Used to order, transfer, or return Schedule I and II controlled substances. Valid for 60 days once executed. Maximum of 20 items per paper form (single-sheet form).

Used to report destroyed or disposed controlled substances (e.g., expired or damaged stock).

DEA Form 106

Used to report significant loss or theft of controlled substances.

DEA Form 224

Required for retail pharmacies, hospitals, clinics, and practitioners to register with the DEA to dispense controlled substances.

Registration for manufacturers, distributors, researchers, and importers.

No product selection indicated; generic substitution allowed and dispensed.

DAW Code 1

Substitution not allowed by prescriber (prescriber wrote "Brand Normally Required" or "DAW").

DAW Code 2

Substitution allowed by prescriber, but patient requested brand product.

DAW Code 3

Substitution allowed; pharmacist selected brand product to dispense.

DAW Code 4

Substitution allowed; generic out of stock, brand product dispensed.

DAW Code 5

Substitution allowed; brand product dispensed at generic price.

Override code used for specific administrative processing.

Substitution not allowed; brand drug mandated by law/state regulation.
