Indications

• FIRST-line drug therapy for HYPERCHOLESTEROLEMIA

• Tx of types lla and llb hyperlipidemias

  REDUCES LDL lvls by up tp 50%

  INCR HDL lvls by 2-5% (getting out and walking biggest change)

  REDUCUSES TRIGLYCERIDES by 10-30%

Mechanism of action

• inhibit HMG-CoA reductase, which is used by the liver to produce cholesterol (endogenous)

• targeting the liver

• lower the rate of cholesterol production

Adverse effects

• Mild, transient GI disturbances (shortly admin)

• rash

• headache

• myopathy (muscle pain) possibly leading to the serious condition RHABDOMYOLYSIS (MOST IMPORTANT)

• Elevations in liver enzymes or liver disease

Wht is Rhabdomlysis?

-Breakdown of muscle protein

-prblm is tht it produces myoglobinuria: urinary elimination of the muscle protein myoglobin. starts to affect renal sys (kidneys) clogging up the tubules.

-CAN LEAD TO ACUTE RENAL FAILURE AND EVEN DEATH

-early=reversible with discontinuation

-immediately report signs of toxicity (muscle soreness or changes in urine color)

Whys is statin drug taken in the evening meal or at bedtime?

ALL STATINS are generally dosed once daily, usually with the evening meal or at bedtime to best concede with the body’s NATURAL diurnal rhythm of CHOLESTEROL PRODUCTION.
