What is primary homelessness?
Doesnt have conventional accomodation (in parks or squatting)
What is secondary homelessness?
Doesnt have accomodation, but has some form of non-permanent shelter (DV shelters, crisis shelters, temporary accom, couch surfing)
What percentage of homeless people have a mental illness?
72-85%, with 50% developing it before becoming homeless, and the other 50% developing it after becoming homeless.
What do the HHOT do?
they provide outreach for people experiencing homlessness using an assertive approach.
What services do the HHOT provide?
The provide a comprehensive mental health assessment, and holistic care through case management.
What is the HHOT service delivery model?
It is a 3 step model: assertive outreach, Case management and collaborative service response.
What is involved with the assertive outreach aspect of the delivery model?
It brings the service to the people in their community setting.
What does the case management aspect of the service model involve?
It embraces a holistic definition of health, and recognises importance of factors that affects one’s health.
What is involved in the collaborative service response?
The co-ordination with other service providers including both government and non-government agencies.
What might assertive outreach look like?
going to where the patient is, seeing them frequently, actively assisting the consumer to acheive their own goals.
What are the different case management styles?
Brokerage: CM organises services required
Collaborative: CM works closely with consumer to achieve goals
Traditional: CM provides all services
What are the roles of case management?
Assist teh consumer to set goals
Assist consumer to achieve goals
Assist consumer to evaluate goals.
