What are goals and outcomes?
Goals are targets that focus on what you want to end with
Outcomes are measurable, specific, and observable results
What are interventions?
Actions that improve, maintain, or restore pt’s health
What are the 3 phases of planning?
1 - initial planning
2 - ongoing planning
3 - discharge planning
What are the 4 ways to prioritize addressing nursing diagnoses?
1 - Maslow’s hierarchy of needs
2 - Patient preference (ex: diet)
3 - Anticipating problems
4 - Clinical reasoning

Cognitive (pt knowledge)

What are the parts of a measurable outcome?
SMART goals or:
1 - subject (who)
2 - verb/actions (observable)
3 - conditions (circumstances)
4 - performance criteria (the estandards)
5 - target time (timeframe)
What are common errors in writing outcomes?
1 - nursing intervention as outcome
2 - non measurable verbs
3 - multiple behavior in one outcome
4 - vague outcomes
What are long term and short term outcomes?
Long term: ultimate goal of care plan
Short term: address acute symptoms or include bite size steps towards long term goal
What are the characteristics of outcomes?
Measurable, patient-centered, evidence based, etc etc
What are the types of nursing interventions?
1 - nurse initiated
2 - physician initiated
3 - collaborative
What are other categories of outcomes beyond the 3 patient learning domains?
1 - clinical outcomes (ex: wound will show no signs of infection by day 5)
2 - functional outcomes (ex: pt will ambulate independently using a walker)
3 - quality of life outcomes (ex: pt will report satisfactory pain control to engage in activites)
