Glasgow Goma Scale
is a neurological tool used to objectively measure a person's level of consciousness, particularly after a traumatic brain injury (TBI). It scores patients from 3 to 15 based on three criteria: eye-opening (1-4), verbal response (1-5), and motor response (1-6).
4 - spontaneous
3 - to speech
2 - to pain
1 - no response
Eye Opening Response (E)
5- oriented (knows who, where, when)
4 - confused (answers questions but disoriented)
3 - inappropriate words (random words, no conversation)
2 - incomprehensible sounds (moaning, no words)
1 - no response
verbal response (V)
6 - obeys commands
5 - localizes to pain
4 - withdraws from pain
3 - flexion to pain (decorticate posturing)
2 - extension to pain (decerebrate posturing)
1 - no response
motor response (M)
mild head injury
GCS 13-15
moderate head injury
GCS 9-12
severe head injury (indicates coma; consider airway protection)
GCS ≤ 8
