what are you testing for using a pen 6-12” from face, have pt follow pen w/ eyes ONLY. Move slowly in a “T”, “X”, & “O” to move thru all areas of visual field.
Document any area/ROM that pt is unable to move into.
Eye ROM (CN III, IV, VI)
what are you testing for using a Snellen chart; pt positioned 20 ft away; test 1 eye at a time; ask to start at top & move down→ lowest line read correctly is visual acuity (can wear glasses)

if move X down & inward, R eye unable to follow what muscle & nerve?

what are you testing for using a pen, have pt track the pen side to side. Do not move to end ROM.
Document smoothness of mvmt & success in tracking

what test of rapid localizations uses 2 targets, ask pt to shift gaze b/t targets on command.
Document any over/undershooting or inability to isolate eye mvmt from head mvmt.

what are you testing for when start w/ a pen 12” from pts nose & ask them to follow tip as you bring pen toward their nose. Observe if both eyes are moving together & if both move (=) in toward nose.
Ask pt to report if/when pen becomes blurry or “double”.
Document distance at blurry/double point→ normal is 2-4”

can get divergence going back out
What are you testing when pt sits in front of examiner & is asked to stare at their nose. Bring a colored object, finger, or pen into their visual field from periphery. Ask pt to report when they see object/finger.
Examiner’s vision is used at “norm”.
Complete in all 4 visual quadrants.
