Nursing Process
A systematic approach to providing nursing care, consisting of five steps: Assessment, Diagnosis, Planning, Implementation, and Evaluation.
Clinical Judgment
The conclusion or decision made regarding a patient's health based on clinical reasoning, assessment data, and patient context.
Assessment
The process of gathering information about a patient's condition, which includes recognizing cues for changes in health.
Hypotonic Solutions
IV solutions that contain a lower concentration of solutes compared to blood plasma, causing cells to swell.
Hypertonic Solutions
IV solutions that contain a higher concentration of solutes compared to blood plasma, causing cells to shrink.
Crystalloid Solutions
Intravenous solutions composed of small molecules that can easily diffuse across semipermeable membranes and are used for fluid replacement.
Fluid Overload
A medical condition that occurs when there is too much fluid in the body, which can lead to heart and kidney complications.
Electrolyte Imbalances
Disruptions in the balance of electrolytes in the body, which can affect numerous body functions and can be life-threatening.
TPN (Total Parenteral Nutrition)
A method of feeding that bypasses the gastrointestinal tract, delivering nutrients directly into the bloodstream.
Proton Pump Inhibitors
Medications that reduce stomach acid production and are commonly used to treat conditions like GERD.
Emergency Contraception
Methods of preventing pregnancy after unprotected sexual intercourse, typically used within 72-120 hours post-intercourse.
ANALYZING CUES
The process of interpreting assessment data to identify patient problems and establish a clinical diagnosis.
Interventions
Specific nursing actions taken to improve patient outcomes based on the assessment and diagnosis.
Priority Actions/Interventions
Immediate nursing actions deemed most important for the safety and care of the patient.
Outcomes
The expected results or goals of nursing interventions, which reflect the patient's health status.
Recognizing Cues
Identifying significant findings during patient assessment that may indicate changes in health status.
