Tools needed for skill:
2 barriers (chuck or towel), toilet paper, hand sanitizer, 2 pairs of gloves, bedpan, graduated container

Bedpan/Urine Output – What is the FIRST step? (opening statement)

explain procedure
check name band
lock wheels of bed
check ID band
Bedpan/Urine Output – After opening statement what do you do?
Put on gloves; prepare supplies
Ask resident to lift their hips; place the barrier towel, then place bedpan correctly (wide part facing buttocks).
Raise the head of the bed to a comfortable level (45 degree angle)
Leave toilet paper and hand sanitizer within reach of resident
place call light nearby (“call me once you’re done”)
Put on gloves (again); Lower the head of the bed
remove the bedpan.
Pour bedpan liquid into graduate on leveled surface (make sure bedpan is on barrier towel)
for state:
(Hold the bedpan for the RN Test Observer while an unknown quantity of liquid is poured into it.)
measure the output (With the graduate at EYE LEVEL)
Clean bedpan; dump, rinse; dump; rinse; dry → place in designated area

Clean graduate; dump, rinse, dump, rinse, dry → place in designated area
Bedpan/Urine Output – After rinsing bedpan, what is the next step?
Ask resident if they need some hand sanitizer
Bedpan/Urine Output – After providing resident with hand sanitizer, what is the next step?
Remove barrier towel (chuck) and dispose
Bedpan/Urine Output – After removing soiled barrier blanket, what is the next step?
Remove gloves and dispose
Record urine output (in mL)
(Must be within 25ml's of RN Test Observer's reading.)
Place call light near resident
Ask if they need anything else
