Bathing Sequence
Always start with the face, then move to upper body, then to lower body.
Elderly Skin Assessment
Skin is prone to tears because of drier skin.
Diabetic Foot Care
Wash and dry feet daily; do not cut nails; do not use lotion between the toes; and check for skin breakdown.
UAP Delegation
The Unlicensed Assistive Personnel is the staff member who should be delegated to give the client a bath.
Infant Sleep Safety
Avoid blankets and other items in the crib due to suffocation hazards; they expect to sleep 10 to 14 hours a day.
Adult Sleep Requirement
7 to 9 hours of sleep each day.
Older Adult (65 Up) Sleep
Expect a decline in sleep patterns.
Dehydration Assessment Signs
Dry mucous membrane, poor skin turgor, oliguria, cool skin, and tachycardia.
Oliguria
Very little urine output.
Oral Hydration Instruction
Offer small sips of water (5,mL); do not force oral fluids. If not tolerated, the next step is an NG tube, not IV fluids.
Normal Urine Output
30,mL an hour.
Specific Gravity
1.016 - 1.022.
Normal Urine pH
4.6 to 8.0.
UTI Gender Prevalence
UTIs are more common in women because the urethra is shorter in women than it is in men.
Anuria
No kidney function.
Glycosuria
Glucose in the urine.
Hematruia
Blood in the urine.
Pyuria
Pus in the urine.
Enuresis
Involuntary urination at night; bedwetting.
24-hour Urine Collection Procedure
Start with an empty bladder (void and throw away the first sample); keep all subsequent urine in one container on ice; if one sample is missed, the process must start over.
Abdominal Assessment Order
Normal Stool Precaution
Universal or standard precautions.
Constipation
Less than 3 bowel movements a week.
Diarrhea
3 or more loose stools in a 24-hour period.
Guaiac Test (Fecal Occult Blood)
Screens for fecal hidden bleeding, which can be an indicator of colon cancer.
Stool Culture and Sensitivity
Determines the best treatment for the bacteria found in the stool.
Enema Administration Position
The client should be in the Lateral Sims Position, not a sitting position.
