Aspirin

324-325mg (Chew), 4 81mg tablets, if alr taken 1, give 3 more etc. FIRST THING TO ADMINISTER before nitroglycerin

Indication - Suspected Acute Coronary Syndrome (ACS) or Chest pain concerning for ACS

Contraindication - Check allergy, not all chest pain is ACS (Could simply be skeletal issue from over lifting a weight, etc)

Diphenhydramine (Benadryl)

Adult: 25-50mg orally, Pediatric (<2yo) 1mg/kg orally, max 50mg

Indication - Mild Allergic Reaction / anaphylaxis

Contraindication - None

Epinephrine (Epi Pen)

Adult: Nasal 2mg if available or intramuscular 0.3mg, Pediatric: Nasal <30kg: 1mg dose spray, >30kg: 2mg spray or IM 0.15mg <25kg, 0.3mg >25kg

Indication - Severe Anaphylaxis (Severe allergic reaction, administer epi and then diphenhydramine)

Contraindication: Medical Control for adults >65 years and children <6months

Albuterol (Inhaler)

Adult: 2.5mg nebulizer for 5 min, Pediatric <2 yrs 1.25, 2+ yrs 2.5-3mg, may repeat up to 3 doses max

Indication: Wheezing in anaphylaxis; has asthma and says that they have their own inhaler


Contraindication: Use with CPAP only if not contraindicated in toxicology

Nitroglycerin

1 tablet or spray (under tongue) every 5 minutes, max 3 doses including the ones they’ve taken that day, SBP must be above 120mm Hg (100mm Hg nationally)


Indication: Chest Pain / suspected acute coronary syndrome (ACS) when the patient has their own prescribed nitroglycerin


Contraindication: Patient is taking erectile dysfunction medications (Sildenafil, Vardenafil, Taldafil, Flolan)
