DASH
The DASH (Disabilities of the Arm, Shoulder and Hand) Outcome Measure is a 30-item, self-report questionnaire that measures physical function and symptoms in people with musculoskeletal disorders of the upper limb.

FABQ
The Fear-Avoidance Beliefs Questionnaire (FABQ) is a patient reported questionnaire which specifically focuses on how a patient's fear avoidance beliefs about physical activity and work may affect and contribute to their low back pain and resulting disability
16 questions 0-6, 96 is highest. First 5 questions have to do with physical activity, the next 11 have to do with work, MCID is 13 points. Score of <19/54 on work sub-scale required for manip guidelines

foot function index
A Foot Function Index (FFI) was developed in 1991 to measure the impact of foot pathology on function in terms of pain, disability and activity restriction. [1] It is a self-administered index consisting of 23 items divided into 3 sub-scales. Both total and sub-scale scores are produced.
Intended Population
The FFI has been shown to be a reasonable tool for use with low functioning individuals with foot disorders[2] ,patients with rheumatoid arthritis[1] and non-traumatic foot or ankle problems.[3] It can be used in both clinical and research settings.
However, it may not be appropriate for individuals who function at or above the level of independence in activities of daily living. [2]

A Global Rating of Change (GRC) score, which is a single-item questionnaire to quantify a change in a certain construct over a specified period of time, was used as an external criterion to assess whether participants had improved, deteriorated or not changed
KOOS
Knee Injury and Osteoarthritis Outcome Score
The Knee Injury and Osteoarthritis Outcome Score (KOOS) is a knee-specific instrument, developed to assess the patients' opinion about their knee and associated problems. The KOOS evaluates both short-term and long-term consequences of knee injury.
Scores range from 0 to 100 with a score of 0 indicating the worst possible knee symptoms and 100 indicating no knee symptoms

Lower Extremity Functional Scale
-Measures function in individuals with lower extremity musculoskeletal conditions.
-Higher scores means higher function.
Minimum score: 0 Maximum score: 80 The lower the score the greater the disability. The Minimal Detectable Change (MDC) is 9 scale points. The Minimal clinically Important Difference (MCID) is 9 scale points.

Oswestry Disability Index
The Oswestry Disability Index (ODI) is calculated based on each score of the ODQ, which consists of ten items. Each of the ten items is scored from 0 to 5, and the total is added and multiplied by 2. Therefore, the ODI ranges from 0 to 100. A higher score on the ODI indicates a more severe disability caused by LBP.

The Patient-Specific Functional Scale (PSFS) is a self-reported, patient-specific outcome measure, designed to assess functional change, primarily in patients presenting with musculoskeletal disorders[1] [2]The scale was developed by Stratford et al 1995 as a self-report measure of function that could be used in patients with varying levels of independence[1]. It was designed to provide clinicians with a valid, reliable, responsive, and efficient outcome measure that would be easy to use and applicable to a large number of clinical presentations

PSS
Penn Shoulder Score
The Penn Shoulder Score (PSS) is a condition- specific self-report measure that first became available in 1999.34 It is a 100-point scale that consists of 3 subscales, including pain, satisfaction, and function. The pain subscale consists of 3 pain items that address pain at rest, with normal activities and with strenuous activities (Appendix). All are based on a 10-point numeric rating scale with end points of ''no pain'' and ''worst possible pain.'' Points are awarded for each item by subtracting the number circled from the maximum of 10. Therefore, a patient is awarded 30 points for complete absence of pain. If a patient is not able to use the arm for normal or strenuous activities, 0 points are scored for that item.

WOMAC Index was developed for Hip and Knee Osteoarthritis, however it has been used with other rheumatic conditions such as: Rheumatoid Arthritis, Juvenile Rheumatoid Arthritis, Fibromyalgia, Systemic Lupus Erythematosus and Low back pain.
