FIBROCYSTIC BREAST DISORDER/CHANGES
what is it?
benign breast condition common in premenopausal women that resolves after menopause
S/S:
heavy breast pain
pain/tenderness in upper outer breast
1wk before period, and resolves 1 wk after period
round, well-defined, freely moveable, pea like
possible dark brown nipple discharge
what are some assessment findings:
changes linked to menstruation
both breast
rarely associated with breast cancer
diagnosis
breast ultrasound to determine if its fluid filled with solid or liquid
fluid filled:
aspiration
fluid analyzed
solid filled:
mammogram ordered
nursing care:
support bra
heat/cold
ibuprofen
FIBROADENOMA
what is fibroadenoma?
noncancerous breast tumor that’s common in young adult females due to increase in estrogen
what are s/s of fibroadenoma?
no nipple discharge
small, round, painless lump
mobile and unilateral
slow growing
doesn’t affect menstrual cycle
what is the diagnosis for fibroadenoma?
mammogram
breast ultrasound
definite diagnosis to rule out breast cancer (fine needle aspiration)
what is the treatment for fibroadenoma?
monitor if small and asymptomatic
surgery if larger than 3 mm and causing symptoms
BREAST DISCHARGE
galactorrhea:
bilateral milky spontaneous
what causes galactorrhea?
increased prolactin
thyroid/pituitary
chest surgery
fibrocystic changes:
bilateral brown/green
what causes fibrocystic changes?
hypersensitivity to estrogen and progesterone
intraductal papilloma:
blood niplle discharge
mammary duct ectasia:
unilateral thick, sticky, white brown, green purple discharge
what is mammary duct ectasia?
inflammation, burning, painful, itchy mass that palpable and irregular
TREATMENT FOR BENIGN BREAST DISORDERS
Intraductal Papilloma
if abnormal cells are found: surgical removal of mass and involved duct
increased risk of breast cancer
Mammary duct ectasia
antibiotics, warm compress, surgical
not associated with breast cancer
MALE GYNECOMASTIA
what is gynecomastia?
benign enlargement of one or both breast because if estrogen and testosterone imbalance
adolescent gynecomastia
temporary hormonal imbalance during puberty that resolves by age 20
treatment:
reassurance
what are 5 causes of gynecomastia?
testicular cancer
adrenal cancer
pituitary tumor
hyperthyroidism
liver disease
what are 5 meds that can cause gynecomastia?
digitalis
isoniazid
ranitidine
spironolactone
marijuana
