Stage 4 Pressure Injury
The stage of pressure injury where expected findings include visible bone.
Primary Intention Healing
A type of wound healing where edge approximation occurs, often involving nutrition considerations for recovery.
Secondary Intention Healing
A type of wound healing that occurs when edges are not approximated, requiring specific nutritional support for tissue repair.
Dehiscence
A complication of wound healing defined as the partial or total separation of wound layers.
Evisceration
A wound complication involving the protrusion of visceral organs through a wound opening.
Granulation tissue
Red, moist tissue in a healing wound that is a key consideration during assessment.
Closed-wound drainage systems
Systems requiring nursing care to clean the port opening, then compress and plug after emptying to maintain suction.
Serous drainage
A type of wound drainage that is clear and watery.
Serosanguinous drainage
A type of wound drainage that is pale, pink, and watery.
Sanguineous drainage
A type of wound drainage that is bright red, indicating active bleeding.
Purulent drainage
A thick type of wound drainage that may appear yellow, green, tan, or brown.
Wound cleansing direction
The practice of cleaning a wound in an outward direction from the least contaminated area to the most contaminated.
Wound Dressings
Common types used in clinical care including gauze, transparent, hydrocolloid, and alginate.
Heat therapy
A therapeutic application that works through vasodilation.
Cold therapy
A therapeutic application that works through vasoconstriction.
Abdominal binder
A supportive device used following surgery for the purpose of compression and support.
Isotonic exercises
Musculoskeletal exercises involving muscle shortening and active movement.
Isometric exercises
Musculoskeletal exercises involving muscle contraction without shortening or active movement.
Incentive spirometry
A pulmonary intervention requiring specific patient teaching on how to use the device to improve lung expansion.
MMSE
The Mini-Mental State Examination, an assessment tool used in nursing care for cognitive impairments like dementia and delirium.
Oxygen devices
Delivery tools including nasal cannula, face mask, Non-rebreather (NRB), Partial NRB, and venturi mask.
Sequential compression device
A cardiovascular nursing intervention used to decrease the physiological impacts of immobility.
COPD / CHF Labs
Key laboratory values used to assess tissue perfusion and oxygenation, specifically Hgb and HCT.
UAP Delegation
Unlicensed Assistive Personnel can collect data such as Vital Signs and I/O, but cannot interpret or trend data.
LPN Delegation
Licensed Practical Nurses are appropriately assigned to care for stable patients only.
