TOF should not be monitored on the affected side because it will give a falsely elevated response (strong twitches may indicate adequate reversal but its not accurate due to the extrajunctional receptors)

extrajunctional receptors= a2b1d1g1 or a7 = more Ach binding sites

depolarizers= increased sensitivity

non depolarizers= decreased sensitivity

since succ / depolarizing NMBD are Ach agonists, there is increased sensitivity of these drugs because there are more Ach receptors.

non-depolarizing NMBD act as competitive antagonists at the Ach receptor. More Ach receptors means NDMR have less effect at the normal dose, there is decreased sensitivity of these drugs.
