Assess the patient if they meet one or more from each criteria

Pt does not obey Commands
SBP < 90
RR < 10 or = or > 30
Need ventilation support
RR < 20 in infant < 1 year
Anatomical Criteria
Any penetrating injuries to head/neck/torso
Penetrating injuries proximal to elbow and knee
Chest wall instability or deformity
2 or more proximal long bone fractures
Amputation proximal to wrist and ankle
Pelvic fractures
Open/depressed skull fracture
Paralysis
Special Criteria
Age; risk of death (+) after age 55 + SBP <110 may represent shock after age 55
Anticoagulation and bleeding disorders
Burns with trauma mechanism
Pregnancy = or > 20 weeks
MOI Falls
Adults: = or > 6m
Children (<15): = or > 3m or 2-3x height of child
MOI Auto Crash
Intrusion = or > 0.3m occupant site + = or > 0.5m any site
Ejection (complete or partial) from vehichle
Death in the same passenger compartment
Vehicle telemetry (damage) consistent with high risk injury
MOI Pedestrian/Cyclist Struck
Thrown, run over, or struck with significant impact (= or > 30km/hr)
MOI Motorcycle Crash
Motorcycle crash = or > 30km/hr
