what is the Nursing process?
a major framework for nursing practice that provides a systematic, person-centered, goal-oriented approach to patient care
what does ADPIE stand for?
assessment, diagnosis, planning, implementation, and evaluation
what happens during the Assessment phase?
collection of both subjective & objective patient data - including patient history, physical examination, and consulting with patient support people
what are examples of data collected during Assessment?
patient history, physical examination, and consulting with patient’s support people
what happens during the Diagnosis phase?
patient data is analyzed, the patient’s strengths & health problems are identified, and nursing diagnoses are formulated
what happens during the Planning phase?
develop an individualized plan of nursing care, establish priorities, select nursing interventions, and communicate with the patient, family, & healthcare team
what happens during the Implementation phase?
Carrying out the plan to assist the patient with achieving desired outcomes; includes collecting data & modifying the plan as needed
what happens during the Evaluation phase?
evaluate how well the patient has achieved the desired outcomes, then decide to either continue, modify, or terminate nursing care
how do the 5 steps of ADPIE relate to each other?
there is great interaction & overlapping among the 5 steps - each step can occur more than once per patient interaction
