Lispro (Humalog)
A rapid-acting insulin with an onset of 10−15min, a peak of 30−90min, and a duration of 3−5hrs.
Aspart (NovoLog)
A rapid-acting insulin with an onset of 10−20min, a peak of 40−50min, and a duration of 3−5hrs.
Regular (Humulin R/Novolin R)
A short-acting insulin with an onset of 30−60min, a peak of 2−4hrs, and a duration of 5−8hrs.
NPH (Novolin N/Humulin N)
An intermediate acting insulin with an onset of 1−2hrs, a peak of 6−12hrs, and a duration of 18−24hrs.
Glargine (Lantus)
A long-acting/basal insulin with an onset of 1−2hrs, no peak, and a duration of 20−24hrs.
SSRI (Selective Serotonin Reuptake Inhibitor)
A class of antidepressants such as Sertraline (Zoloft) and Citalopram (Celexa) that increase serotonin level; key points include 4−6wks onset, AM administration, and risks of suicide and serotonin syndrome.
TCA (Tricyclic Antidepressant)
A class of antidepressants such as Amitriptyline that affects serotonin and norepinephrine; it has a 2−3wks onset, should be avoided in cardiac or elderly patients, causes anticholinergic side effects, and requires a 14-day washout from MAOIs.
MAOI (Monoamine Oxidase Inhibitor)
A class of antidepressants such as Phenelzine (Nardil) that blocks the MAO enzyme; it requires a 4wk onset, avoidance of tyramine foods to prevent hypertensive crisis, and a 14-day washout before other antidepressants.
DKA (Diabetic Ketoacidosis)
A condition occurring mostly in Type 1 diabetics with abrupt onset, BG of 300−500, ketones, acidosis, fruity breath, and Kussmaul respirations; treated with IV fluids and insulin, switching to D5W when BG is 250−300.
HHS (Hyperglycemic Hyperosmolar State)
A condition occurring mostly in Type 2 diabetics with gradual onset, BG >600, no ketones or acidosis, and predominant neurological changes.
