What is the goal of the Collect step?
To collect necessary subjective and objective information about the patient in order to understand the relevant medication history and clinical status
What is the purpose of the patient interview?
1. To ensure they have the most accurate and up-to-date medication list
2. To assess drug-related needs
3. To identify drug therapy problems
4. To develop a therapeutic relationship
Give some example questions regarding asking about the patient's Chief Complaint
What brings you into clinic today?
Tell me how well you new medications have been working for you
What would you like me to help you with?
Give some example questions regarding asking about the patient's History of Present Illness
How long has this been happening?
When did it start?
Has anything made it better or worse?
Give some example questions regarding asking about the patient's Past Medical History
