• snack

• 50% of food eaten

• 50% fluid intake

opening statement

• introduce yourself, greet resident by name

• explain you’re there to help them eat

• close curtain

• go wash hands

• gather materials: clothing protector, hand wipe, chair, paper towel, measure sheet, pen

• bring chair into room

• fill resident’s cup with water + get paper towel at sink

prepping the resident

• offer resident clothing protector

• tell them to sit up straight

• take a seat

• prepare the food: open straw w/o touching part that goes in drink + open snack

• place trash on paper towel

• offer resident wet wipe w/o touching it —> have them dispose on paper towel

• take all trash to trash can

procedure

• open spoon (don’t touch side that goes in mouth) + small bites

• TALK WITH RESIDENT

• offer sips of water after every 2-3 bites

• pat mouth dry after each sip of water

• keep going until resident refuses you

• offer them another snack or to save it for later

make mental note of how much they ate/drank bc you need to chart this

clean up

• remove clothing protector and take to hamper

• take the rest of food/drink to trash

• put the chair back where they got it

• RECORD/CHART: must be within 25% of nurse + sign at bottom

closing statements

• ask if you can provide them with a magazine/tv (IC)

• place call light in hand

• open curtain

• wash hands
