12-lead EKG placement - number of electrodes placed
10 electrodes - 4 limb ; 6 precordium (chest)
limb leads/electrode placement - 3,5, and 12 lead EKGs
right arm, left arm, right leg, left leg (can also be place on right/left chest/abdomen areas)

V1 placement - 12-lead EKG
4th intercostal space - right side of patients sternum

V2 placement - 12-lead EKG
4th intercostal space - left side of patients sternum

V3 placement - 12-lead EKG
between V2 and V4

V4 placement - 12-lead EKG
5th intercostal space - left side of patients sternum

V5 placement - 12-lead EKG
5th intercostal space - patients left side - midclavicular line

V6 placement - 12-lead EKG
5th intercostal space - patietns left side - midaxillary line

where is the singular non-limb electrode placed for a 5-lead EKG
in the middle of the chest - just below the patients suprasternal notch (can also be placed on patients right side - 4th intercostal space)

Chest pain/discomfort suggestive of cardiac ischemia
Epigastric pain
Shortness of breath
Syncope (or near-syncope)
Pulmonary edema/Cardiogenic shock
Wide complex tachycardia
Symptomatic bradycardia
Stroke
Altered level of consciousness (ALOC)
Vague “unwell” symptoms in diabetic and elderly patients.

turn on the monitor
hit “12-lead” button on the side panel
input the patients age and gender
hit “acquire 12-lead” - INSTRUCT PATIENT TO REMAIN STILL
after 10-seconds, patient may relax, after “requiring 12-lead” shows on monitor

right limb lead placement - for all EKGs
“clouds above grass” - RA = white lead ; RL = green lead

left limb lead placement - for all EKGs
“smoke above fire” - LA = black lead ; LL = red lead
