A 24-year-old woman presents to the emergency department with agitation, confusion, and muscle stiffness. She has a history of depression treated with sertraline. Three days ago, she began taking tramadol for back pain prescribed at an urgent care clinic.

On examination, her temperature is 39.4°C (103°F), heart rate is 120 bpm, and blood pressure is 165/95 mmHg. She appears restless and diaphoretic. Neurologic examination reveals hyperreflexia and inducible clonus in the lower extremities. Tremor is also noted.
