What are the effects of tenecteplase?

A thrombolytic drug that breaks up thrombi and emboli in the body

What is the mechanism of action of tenecteplase?

- Binds to fibrin-rich clots via the fibronectin finger–like domain and the Kringle 2 domain
- The protease domain then cleaves the Arg/Val bond in plasminogen to form plasmin
- Plasmin in turn degrades the fibrin matrix of the thrombus, thereby exerting its thrombolytic action

What are the contraindication of tenecteplase?

- Fibrinolytics (medications used in the treatment of dissolving intravascular clots)
- Acute pancreatitis
- Aneurysm
- Coma
- Heavy vaginal bleeding
- Pericarditis
- Gastrointestinal ulceration
- Recent haemorrhage (including dental extraction)
- Severe hypertension
- Recent trauma
- History of cerebrovascular disease

What are the side effects of tenecteplase?

- Bleeding (major and minor)
- Arrhythmias
- Anaphylaxis
- Hypotension
- Hemiparesis

When should tenecteplase be used?

When an event of acute ST elevation MI within 6 hours of onset of symptoms where PPCI is not available and the patient is able to have it according to the checklist
