Catheter Care for a female with hand washing
Hold catheter where it exits the urethra with one hand, clean with strokes only away from the urethra (at least twice), wash hands - don't recontaminate hands by touching faucet/sink
Isolation gown and gloves then emptying a urinary drainage bag with hand washing
Candidate's measured output reading within 25ml of RN, don't re-contaminate hands by touching the faucet or sink at any time during/after hand washing procedure
Perineal care of a female with hand washing
Using a clean portion of a wash cloth, clean the vaginal area from top to bottom; clean from vagina to rectal area; don't re-contaminate hands by touching the faucet or sink at any time during/after hand washing procedure
Dressing a bedridden resident
When dressing the resident in a button-up shirt, always dress from affected (weak) side first; when dressing the resident in pants, always dress from the affected (weak) side first
Feeding a dependent resident
Food intake is within 25% of RN and fluid intake is within 60ml of RN
Foot Care one foot
Dry thoroughly between toes
Mouth Care - brushing teeth
Brush teeth, including inner, outer, and chewing surfaces of upper and lower teeth
Pivot-transfer from bed to wheelchair and/or Pivot-transfer from wheelchair to bed
Verbalize that this is a 1-person transfer per the resident's care plan, lock bed brakes, lock wheelchair brakes
Position resident on side
Direct the RN to stand on the side opposite working side or raise rail on opposite side
Range of Motion for Hip and Knee and/or Range of Motion for Shoulder
Must ask at least once if there is pain/discomfort during each ROM exercise
Vital signs
Pulse within 4 of RN, respirations within 2 of RN
