V1
Fourth ICS, right sternal border. The positive electrode is placed directly over the right atrium.
V2
Fourth ICS, left sternal border. The positive electrode is placed just anterior to the atrioventricular (AV) node.
V3
Halfway between V2 and V4. The positive electrode is placed over the ventricular septum.
V4
Fifth ICS, left midclavicular line. The positive electrode is placed over the ventricular septum but to the left of V3.
V5
Fifth ICS, left anterior axillary line. The positive electrodes are placed over the lateral surface of the left ventricle.
V6
Fifth ICS, left midaxillary line. The positive electrodes are placed over the lateral surface of the left ventricle.
QRS Complex in V1
Mainly negative, and the R wave is small because the flow of electricity moves away from the positive electrode.
QRS Complex in V2
Mainly negative, and the R wave is slightly taller than in V1.
QRS Complex in V3
Biphasic, which means half is above the baseline and half is below the baseline.
QRS Complex in V4
More positive than in V3.
QRS Complex in V5
More positive than in V4.
QRS Complex in V6
More positive than in V4.
Manubrial notch
The notch at the top of the sternum.
Angle of Louis
The angle formed by the junction of the manubrium and the body of the sternum.
