What is evaluating?
Nurse and pt measure how well the pt has achieved the outcomes in their care plan
What does it mean to terminate, modify, or continue a care plan?
You decide the next steps for the care plan based on the evaluation findings:
Terminate - finish it if outcome is met or no longer appropriate
Modify - adjust the plan
Continue - keep doing plan
Describe the 4 types of outcomes and how we evaluate these outcomes
1 - cognitive: increase in pt knowledge (have pt repeat info back to you or explain it)
2 - psychomotor: achievement of new physical skills (observe if they can use new skill)
3 - affective: changes in pt values, beliefs, attitudes (ask pt about these domains)
4 - physiologic: physical changes in pt (measure vitals)

1 - decide level of achievement (met, partially met, not met)
2 - support with pt data (behaviors or data points)
How can you make revisions in care?
You can revise:
diagnosis
time criteria
outcome
interventions
Explain ways to improve the care plan and nursing care during evaluation?
Follow steps similar to the nursing process
discover problem
assess and adjust
plan with indicators
implement change
