Assessment Items GG 0100

The Section GG category evaluating Prior Functioning: Everyday Activities prior to the current illness, exacerbation, or injury.

Prior Functioning: Self-care (GG 0100A)

The ability to use suitable utensils to bring food to the mouth and swallow food or liquid once the meal is placed before the patient prior to illness/injury.

Prior Functioning: Stairs (GG 0100C)

Code for the patient's need for assistance with internal or external stairs (with or without a device) prior to current illness, exacerbation, or injury.

Prior Functioning: Functional Cognition (GG 0100D)

Code for the patient's need for assistance with planning regular tasks, such as shopping or remembering to take medication, prior to current illness/injury.

Assessment Items GG 0110

The Section GG category indicating devices and aids used by the patient/resident prior to the current illness, exacerbation, or injury.

Assessment Item GG0115

Functional Limitation in Range of Motion; codes for limitation that interfered with daily functions or placed resident at risk of injury in the last 7 days (utilized on MDS).

ROM Limitation: Upper Extremity (GG0115A)

Range of motion limitation interfering with function or creating injury risk in shoulder, elbow, wrist, or hand.

ROM Limitation: Lower Extremity (GG0115B)

Range of motion limitation interfering with function or creating injury risk in hip, knee, ankle, or foot.

Assessment Item GG0120

Mobility Devices; records all mobility devices normally used by the patient in the last 7 days (utilized on MDS).

Assessment Items GG 0130

The Section GG assessment category covering Self-Care Items evaluated at Admission, Goal, and Discharge.

Eating (GG 0130A)

The ability to use suitable utensils to bring food to the mouth and swallow food/ or liquid once the meal is placed before the patient.

Oral Hygiene (GG 0130B)

The ability to use suitable items to clean teeth, or insert, remove, soak, and rinse dentures with use of equipment.

Toileting Hygiene (GG 0130C)

The ability to maintain perineal hygiene and adjust clothing before and after voiding/bowel movement; includes wiping ostomy opening but not managing equipment.

Wash Upper Body (GG 0130D)

The ability to wash, rinse, and dry the face, hands, chest, and arms while sitting in a chair or bed (reported to CMS only in LTCH).

Shower/Bathe Self (GG 0130E)

The ability to bathe self, including washing, rinsing, and drying self (excludes back, hair, and tub/shower transfers; not reported in LTCH).

Upper Body Dressing (GG 0130F)

The ability to dress and undress above the waist, including fasteners if applicable (not reported to CMS in LTCH).

Lower Body Dressing (GG 0130G)

The ability to dress and undress below the waist, including fasteners; does not include footwear.

Putting On / Taking Off Footwear (GG 0130H)

The ability to put on and take off socks and shoes or other footwear appropriate for safe mobility, including fasteners if applicable.

Personal Hygiene (GG 0130I)

The ability to maintain personal hygiene, including combing hair, shaving, applying makeup, and washing/drying face and hands (reported only on MDS).

The Section GG assessment category covering Mobility Items scored at Admission, Goal, and Discharge.

Roll Left and Right (GG 0170A)

The ability to roll from lying on back to left and right side, and return to lying on back on the bed.

Sit to Lying (GG 0170B)

The ability to move from sitting on side of bed to lying flat on the bed.

Lying to Sitting on Side of Bed (GG 0170C)

The ability to safely move from lying on the back to sitting on the side of the bed with feet flat on the floor, and with no back support.

Sit to Stand (GG 0170D)

The ability to safely come to a standing position from sitting in a chair or on the side of the bed.

Chair/Bed-to-Chair Transfer (GG 0170E)

The ability to safely transfer to and from a bed to a chair (or wheelchair).

Toilet Transfer (GG 0170F)

The ability to safely get on and off a toilet or commode.

Tub/Shower Transfer (GG 0170FF)

The ability to safely get in and out of a tub or shower (reported only on MDS).

Car Transfer (GG 0170G)

The ability to transfer in and out of a car or van on passenger side (excludes opening/closing door or fastening seat belt; not reported in LTCH).

Walk 10 Feet (GG 0170I)

Once standing, the ability to walk at least 10 feet in a room, corridor, or similar space.

Walk 50 Feet with 2 Turns (GG 0170J)

Once standing, the ability to walk at least 50 feet and make two turns.

Walk 150 Feet (GG 0170K)

Once standing, the ability to walk at least 150 feet in a corridor or similar space (not reported in LTCH).

Walking 10 Feet on Uneven Surfaces (GG 0170L)

The ability to walk 10 feet on uneven or sloping surfaces (indoor or outdoor), such as turf or gravel.

1 Step (Curb) (GG 0170M)

The ability to go up and down a curb and/or up and down one step (can score if using wheelchair; not reported in LTCH).

4 Steps (GG 0170N)

The ability to go up and down four steps with or without a rail (can score if using wheelchair; not reported in LTCH).

12 Steps (GG 0170O)

The ability to go up and down 12 steps with or without a rail (can score if using wheelchair; not reported in LTCH).

Picking Up Object (GG 0170P)

The ability to bend/stoop from a standing position to pick up a small object, such as a spoon, from the floor (not reported in LTCH).

Code 6 (Independent)

Score indicating the patient/resident safely completes the activity by themselves with no physical, verbal/nonverbal cueing, setup, or clean-up assistance.

Code 5 (Setup or Cleanup Assistance)

Score indicating helper sets up or cleans up while patient completes activity; helper assists only prior to or following activity.

Code 4 (Supervision or Touching Assistance)

Score indicating helper provides verbal cues and/or touching/steadying/contact guard assistance throughout or intermittently during activity.

Code 3 (Partial/Moderate Assistance)

Score indicating helper provides physical assistance (e.g., lifting/supporting trunk or limbs) with helper providing less than half of the effort.

Code 2 (Substantial/Maximal Assistance)

Score indicating helper provides physical assistance (e.g., lifting/supporting trunk or limbs) with helper providing MORE THAN HALF of the effort.

Code 1 (Dependent)

Score indicating helper does ALL (100%) of the effort, patient does none, OR assistance of 2 or more helpers is required at any time.

Code 88 (Not Attempted - Medical Condition/Safety)

Section GG performance code used when an activity was not attempted due to medical condition and safety concerns.
