Republic Act No. 4688
Also known as the Clinical Laboratory Law of 1966, approved on June 18, 1966, regulating the operation and maintenance of clinical laboratories in the Philippines.
RA 4688 Section 1 Exemption
Government hospital laboratories performing routine or minimum laboratory examinations whose services serve as extensions of government regional or central laboratories.
RA 4688 Section 4 Penalties
Imprisonment of not less than 1 month nor more than 1 year, and/or a fine of not less than 1,000 pesos nor more than 5,000 pesos.

The regulation titled 'New Rules and Regulations Governing the Regulation of Clinical Laboratories in the Philippines'.
Clinical Pathology
A functional laboratory classification covering clinical chemistry, clinical microscopy, toxicology, therapeutic drug monitoring, immunology and serology, hematology and coagulation, bacteriology, parasitology, mycology, and virology.
Anatomic Pathology
A functional laboratory classification covering surgical pathology, cytopathology, immunohistochemical techniques, autopsies, and forensic pathology.
Molecular Pathology
A functional laboratory classification covering molecular procedures such as PCR.
Primary Service Capability
Provides minimum clinical services including Urinalysis, Fecalysis, FOBT, Pregnancy Test, Wet Smear, FBS/RBS, OGTT, Lipid Profile, Creatinine, BUN, BUA, CBC, ABO/Rh typing, and rapid tests for Dengue, Syphilis, Hep B, and HIV.
Secondary Service Capability
Provides primary capabilities plus Coagulation studies (PT, aPTT for hospital-based), Serum Electrolyte panel (Na, K, Cl), ALT, AST, Gram Stain, KOH, and Pap smear.
Tertiary Service Capability
Provides secondary capabilities plus other Clinical Chemistry tests, Arterial Blood Gases (hospital-based), automated serology/immunology, culture and sensitivity, and Cytology/Histopathology (hospital-based).
Limited Service Capability
Provides 1 or 2 specialized tests not under Anatomic or Molecular Pathology (e.g., hormones, trace metals, tumor markers, allergy panel) or DOH-program related tests (e.g., Kato Katz, Malarial smear, Filaria smear, Slit-skin smear, RPR).
DOH Permit to Construct (DOH-PTC)
A permit required prior to constructing a new clinical laboratory or for renovation, expansion, change in ownership, or location transfer of an existing laboratory.
DOH License to Operate (DOH-LTO)
A non-transferable license issued by HFSRB/CHD-RLED valid for 1 year, requiring annual renewal and prominent display to the public.
Certificate of Registration (COR)
A regulatory document required for clinical laboratories operated and maintained exclusively for research and teaching purposes, valid for 3 years.
Non-Institution Based CL Offense Fines
Sanctions for violations: 1st offense = stern warning; 2nd offense = Php 30,000; 3rd offense = Php 50,000; 4th offense = Revocation of DOH-LTO.
Unlicensed Clinical Laboratory Penalty
Operation without DOH-PTC and DOH-LTO results in a Cease and Desist Order and an administrative penalty of Php 50,000.

Primary Clinical Laboratory Sample Floor Plan
A floor plan scheme for non-institution-based primary clinical laboratories with a gross floor area of 62m2.

Secondary Clinical Laboratory Sample Floor Plan
A floor plan scheme for non-institution-based secondary clinical laboratories with a gross floor area of 110m2.

Tertiary Clinical Laboratory Sample Floor Plan
A floor plan scheme for non-institution-based tertiary clinical laboratories with a gross floor area of 160m2.

Minimum Area Requirements for Collection and Administrative Space
Mandatory space allocations: Blood Extraction Area (6.00m2/chair or couch), Specimen Collection Toilet (1.67m2), Pathologist Area (5.02m2/staff), and Waiting Area (0.65m2/person).

Biosafety and Biosecurity Officer Qualifications
Requires PRC certificate and valid PRC ID as Registered Medical Technologist, plus Certificate of training in Biosafety and Biosecurity from RITM and/or UP-NTCBB.

Registered Medical Technologist Licensing Evidence
Required documentation includes updated resume, PRC certificate with valid ID, training certificates, notarized employment contract, annual medical certificate, and proof of Hepatitis B and Influenza immunization.

Molecular Pathology Analyst Staffing Requirement
Requires 3 analysts per shift, all trained in molecular diagnostics, with at least 1 per shift trained in Biosafety/Biosecurity/Biorisk Management.
False Test Results Penalty
Any person authorized or licensed who issues false laboratory test results knowingly, willfully, or through gross negligence shall be permanently disqualified from owning, managing, operating, or analyzing in any DOH-licensed CL.
