wide QRS > 0.12 sec and focus on V1/V6 leads
Right BBB
* V1 rsR’ (basically R is upwards)
V6 would have a slurred S wave
Left BBB
V1 QS/rS complex (basically R is downwards)
V6 tall R wave, no Q, RSR’
Left axis deviation
Brugada syndrome
narrow QRS
V1-V3 RSR’ with ST elevations with negative T waves
Atrial Hypertrophy
P wave
Left Atrial Hypertrophy(p-mitral)
Lead I / II “m” shaped p wave > 0.12 sec
confirm with V1 Biphasic P(second half bigger for left hypetrophy)
Right Atrial Hypertrophy (p-pulmonale)
II / III peaked p waves > 2.5mm height
confirm with V1 Biphasic P(first half bigger for Right hypertrophy)
Ventricular Hypertrophy
V1/2 and V5/6
Left Ventricular Hypertrophy
Deepest S wave in V1/2 + Tallest R wave in V5/6 > 35
Right Ventricular Hypertrophy
V1/2 tall R > 7mm
V5/6 deep S > 7mm

Lateral MI
I, avL, V5/6. MC left circumflex artery

Inferior MI
II, III, avF……MC right coronary artery

Anterior MI
V3/4: left anterior descending artery

Septal MI
V1/2: left anterior descending artery

Posterior MI
Anterior wall MI(V3/4) with ST depression….right coronary artery or left circumflex artery
