Collaboration
Cooperation among healthcare professionals from various disciplines who bring knowledge, skills, and attitudes from their profession to provide optimal care for the client.
Collaborative health care
A client-centered approach in which members of different healthcare professions come together and work toward a common goal of improving or restoring a client’s health.
Silos
A term describing when healthcare professionals are educated solely within their own professional discipline, which can perpetuate medical errors.
Interprofessional Teams
Two or more professionals working together to improve access to and coordination of healthcare services.
Care coordinator
The role the nurse serves when assisting to organize an interprofessional team to ensure the needs of the client are met.
TeamSTEPPS®
An evidence-based interprofessional strategy standing for Team Strategies, Tools to Enhance Performance and Patient Safety.
Consultation
When one provider formally requests another provider’s input or advice as to how to approach or treat a condition.
Referral
When one provider delegates responsibility for the management of a client’s specific condition to another provider.
Transfer of care
The process of systematically turning care over to another provider.
Interpersonal communication skills
Skills used to build strong relationships that help improve performance, job satisfaction, and social skills.
Five rights of delegation
The framework consisting of Right task, Right circumstance, Right person, Right direction and communication, and Right supervision and evaluation.
I-SBAR-R
A standardized communication tool for the transfer of care consisting of Introductions, Situation, Background, Assessment, Recommendation, and Readback.
Receive-Record-Read Back
The process for receiving a prescription: record it, read it back as written to the prescriber, and verify.
Interprofessional Education Collaborative (IPEC)
An organization that developed core competencies for teamwork addressing Values and Ethics, Roles and Responsibilities, Interprofessional Communication, and Teams and Teamwork.
Mutual support
A teamwork skill where a team member provides feedback and assistance to the team on how to be more efficient team members.
