Vocabulary terms and definitions detailing radiological documentation types, program regulatory standards, and procedure execution classifications from SATA1374A Module 1.0.
10 CFR Part 835 (1998)
Federal regulation that establishes radiation protection standards, limits, and program requirements for protecting individuals from ionizing radiation resulting from the conduct of DOE activities.
DOE Directive DOE G 441.1-11
The Department of Energy guidance document titled "Occupational Radiation Protection Record-Keeping and Reporting Guide" referenced for radiological record-keeping standards.
Employment History Records
Radiological control records detailing an employee's previous and ongoing radiological work assignments and yearly radiation doses at DOE and non-DOE facilities, maintained to preserve dose-to-job function correlation for trend analyses and future health studies.
Personnel Radiological Records
Occupational radiation dose records maintained for contractor, subcontractor, and Federal employees in the personnel dosimetry program, including external doses, internal specimen/dose assessments, radiological incident reports, and counseling documentation.
Medical Records (Radiological Control)
Records comprising reports of periodic medical examinations and evaluations, respirator fit-testing results, and medical treatment performed in support of the radiological control program.
Radiological Training and Qualification Records
Permanently maintained records verifying that an individual received appropriate training to perform work safely, including qualification standards for on-the-job, practical, and classroom training, as well as quizzes, tests, dates, and trainee signatures.
Instrumentation and Calibration Records
Maintained records documenting the calibration, modification, maintenance, and periodic operational checks of radiation measuring equipment, including source traceability, frequencies, and special tests conducted following suspected overexposures.
Controlled Copies
Official copies of laboratory-wide manuals and procedures maintained in document control that must be verified as the current revision prior to use to prevent being treated as uncontrolled or information-only copies.
Deficient Procedure Protocol
The required three-step action taken when a procedure is deficient or cannot be followed as written: 1. Stop the operation; 2. Notify the cognizant manager or supervisor; 3. Initiate a procedure change or revision.
Use Type 1 Procedures
Procedures for which failure to comply in a step-by-step manner could result in a significant health, safety, or environmental risk to the employee or public, or could have a significant adverse impact on facility operations.
Type 1 Procedure Execution Requirements
Operational requirements mandating that procedures are issued each time performed, verified current upon issue, kept in the physical possession of the user, followed sequentially step-by-step, and have steps signed off or initialed as performed.
Use Type 2 Procedures
Procedures for which failure to comply in a step-by-step manner could result in a health, safety, or environmental risk to the employee or public, or could have an adverse impact on facility operations.
Type 2 Procedure Execution Requirements
Operational requirements mandating that procedures are verified current prior to use, available in the immediate work area, referenced as necessary, and followed sequentially step-by-step without requiring sign-offs or initials.
Use Type 3 Procedures
Procedures that provide administrative direction and define activities necessary to carry out programs, where failure to comply step-by-step results in little or no risk or facility impact; this category encompasses most radiological control procedures.
Use Type 4 Procedures
Procedures intended for frequently performed, relatively simple technical activities within the skill and knowledge level of experienced and trained individuals, which can be performed from memory.
